Facility Evaluation Report

LPA Hiratsuka conducted this unannounced annual visit. Most of the clients were participating in outings with staff during LPA's visit. There were three clients and the administrator present.

This facility opens to the main common area. There is an office to the left of the main entrance. There are two client restrooms and one staff restroom. There is a separate building on the property that is used for meetings. There is a green house in the backyard. This facility has a kitchen in the back.

Two client records and staff training records were reviewed.
