Licensing Program Analyst Hiratsuka conducted this unannounced case management visit in response to an incident the administrator reported on June 26,2025. LPA accidentally linked the incident to this facility. The incident occurred at the sister facility ANGEL WINGS, INC. #2 facility # 585002598. Administrator gave permission to conduct the visit at this facility since all the residents were at day program.
LPA reviewed some of the paperwork in the resident's file. LPA interviewed Administrator.
