On 7/10/2026, Licensing Program Analyst (LPA) Nakagawa arrived unannounced to conduct a case management inspection and met with Administrator Alka Ralh to discuss.
LPA is following up regarding the reported death of resident, R1. LPA reviewed documents. On 05/29/2026, Administrator met with family of R1 to discuss hospice enrollment; family and facility were waiting on return of DPOA from trip and was expected within one week. R1 passed on 06/03/2026. Administrator has agreed to provide LPA a copy of the death certificate once it is available.
No deficiencies cited during this inspection.
