Facility Evaluation Report
Licensing Program Analyst (LPA) Jill Nakagawa arrived unannounced to conduct an Annual Inspection. The ARF has 6 residents, who were off site at the time of visit, attending their day programs. There were 3 staff present.
Licensee Andrew Gamboa received LPA at the door. Administrator arrived shortly. LPA reviewed 3 of 6 resident files and one new staff member's personnel file and found all to be complete. All 6 staff members have a valid First Aid/CPR card and have fingerprint clearance.
Licensee then took LPA on a tour of the facility. The facility has a large common room with seating for all the clients to relax and socialize. The room was decorated for Christmas at the time of visit. Clients' rooms are furnished as per regulation and include personal touches. Bathrooms have required non-skid mats and grab bars. There is a hard-wired fire alarm system and 1 carbon monoxide detector, which was tested and operational. Fire extinguisher, last serviced on 09/25. The water temperature measured 109.5 degrees F. The last fire drill was on 11/13/2025. Toxins are locked in cabinets in the garage. There is a month's supply of medications for each resident, which are stored in a locked medication cabinet. There was an ample supply of perishable and non-perishable food as required per regulation. The back yard provides a nice space for clients to exercise, garden and relax. The yard was free of obstructions and debris.
Discussion was had regarding medication storage.
No deficiencies during today's inspection.
