Facility Evaluation Report
On 08/19/2025, Licensing Program Analyst (LPA) Jill Nakagawa arrived unannounced to conduct an Annual Inspection. There are currently 9 clients at the facility. At the time of inspection there were 5 staff on site.
LPA was allowed entry by staff and logged in. LPA met with Administrator Sarah Shepherd,
LPA and Administrator toured the facility Facility was clean and orderly, and a comfortable temperature. Residents' rooms were clean and had the required furnishings. Bathrooms were clean and sanitary, and equipped with new shower curtains, grab bars, non-slip mats and supplied with paper towels and hand soap. Living room was furnished with new couches and provided a homey atmosphere. Clients enjoy the comforts of the large screen TV, listen to music, and lots of arts and crafts and reading materials. Clients have access to phone and internet. Back patio had lots of comfortable seating; and there is corn hole and basketball. Fire extinguishers were last serviced on 10/15/2024. Medications and sharps were locked and inaccessible to clients. The kitchen had an ample supply of perishable and non-perishable food and snacks available at all times. The kitchen floor was recently renovated.
The facility also provides a private visitation area and County caseworkers and clinicians. Staff are receiving monthly training through Health Stream.
A review of 3 client and 3 staff files found them to be complete.
LPA requested updated copies of the LIC 500, Control of Property, Liability Insurance, Fire Alarm testing.
There were no deficiencies found at the time of this inspection.
