Facility Evaluation Report
On 03/27/2026, Licensing Program Analyst (LPA) Jill Nakagawa arrived unannounced to conduct an Annual Inspection and met with Senior Program Director, Nati Mauga. There were 18 clients with six (6) staff on site at the time of visit.
LPA arrived at 9:15 AM to find clients arriving to their Day Program. Each client brings their lunch and knows the routine for arrival. There is currently one (1) client with Virtual Program participation. The facility was found to be clean and at a comfortable temperature with all exits free from obstruction. Water temperature in the kitchen and two (2) bathrooms was within regulation of 105-120 degrees F. There are two fire extinguishers which were fully charged and last serviced on May 7, 2025. There were two carbon monoxide detectors which were tested and found to be operational. Fire Drills are conducted each month; the last one was conducted on March 24, 2026. Cleaning supplies and other toxins, fluorescent light bulbs and art supplies were locked in a closet inaccessible to clients. The kitchen has one (1) refrigerator which is used for clients to store their lunches, in addition to the supplies needed for cooking activities. Sharps are stored inaccessible to clients. There is a designated client break room for any client who needs a quiet space. Other areas for clients to use: small room for reading, drawing, yoga or individual class, a group room for watching movies, exercise, and other group activities. There is an outside area in the back of the building that has an area for basketball, gardening projects, picnics. The facility has a large canopy for shelter during activities during the warm, sunny weather. The facility was full of activities going on that involved each and every client.
