Facility Evaluation Report
On 01/30/2026, Licensing Program Analyst (LPA) Jill Nakagawa arrived unannounced to conduct a Case Management visit at this facility regarding an incident report that was self-reported to CCL on January 14, 2026 and met with Regional Program Supervisor, Nati Mauga. There were 21 clients with eight (8) staff on site at the time of visit.
On 01/13/2026, at approximately 12:50 PM Client (C1) was eating their lunch in the main room in their preferred seat under the observation of staff (S1). S1 observed that C1 began showing signs of choking. S1 immediately assessed the situation and began emergency response procedures and administered three abdominal thrusts which dislodged the piece of apple that C1 had been eating.(Clients bring their own lunches each day.) At 12:52 PM Staff (S2) asked how they were doing. C1 told S2 that they were fine, they had just choked on their apple. S2 gave C1 a verbal reminder to slow down and chew their food. These reminders are given to C1 regularly as safe eating practices are one of C1's goals. C1 is also supervised while eating. C1 was able to finish their lunch with no ill effects. Facility made all appropriate notifications per regulation.
