The inspector’s account

On 08/28/2025 at approximately 9:35AM Licensing Program Analyst (LPA) Jill Nakagawa arrived unannounced to conduct an Annual Inspection. There were two care staff and two residents at the time of inspection. Administrator Ramon Silvestre was called upon LPA's arrival and arrived shortly.

The facility was clean and well-organized. Residents appeared well-cared for and stated they were happy with their care and the food was good. Required postings were present on hallway at entrance of facility. The kitchen was clean and sanitary. Sharps were stored inaccessible to residents in care. There was an ample supply of fresh and non-perishable foods and food was stored and dated as required per regulation. The bathrooms were clean and sanitary and had the required grab bars and non-skid surfaces. The facility provides a land line phone for residents to stay in touch with family and friends. There was a newly purchased Fire Extinguisher that was charged and operational. The internal fire alarm system was last tested by Fire Department on 02/05/2025 and the last fire drill was conducted on 06/05/2025.

LPA conducted review of two resident files and four staff files and found them to be complete.

There were no deficiencies found at the time of inspection. No citations issued.
