The inspector’s account

Licensing Program Analyst (LPA) Jill Nakagawa conducted an unannounced Annual inspection on 06/25/2025 at approximately 10:05 AM. LPA is conducting visit with Maritza Salazar, Administrator. There are currently 15 clients in care. There were 2 staff in addition to the Administrator on site at the time of inspection.

The facility's common areas were clean and well-organized and a comfortable 71 degrees F. Client rooms were organized, had required furnishings and linens. Administrator stated there were 2 dressers they were working on replacing. LPA observed several clients in the main common area watching television. There was also exercise equipment, karaoke machine, computer and video games available for client use. Other clients were preparing for a trip to the store.

There was an ample supply of of fresh and nonperishable foods as required in Title 22 regulation. There is a drink dispenser on the counter in kitchen providing hydration. Medications were observed locked in medication room. Toxins were observed locked in a small restroom, accessible by staff only. Bathrooms were equipped with necessary grab bars and non-slip floors/mats and had soap and paper towels and were sufficiently supplied with other hygiene products. The facility has 3 fire extinguishers which were last inspected on 04/30/2025 and were fully charged. 2 carbon monoxide detectors were tested and operational at the time of inspection. The last disaster drill was conducted on June 19, 2025 for the day shift. The fire department conducted an inspection on 06/15/2025 and found no violations.

LPA spoke with 5 out of 15 clients, all of whom reported that the Administrator, Licensee and staff provided them with good care and food.

LPA reviewed records of 5 clients and found 5 of 5 to be complete; 5 of 5 staff records were reviewed. All were complete but required the training to be included in facility folders rather than just corporate HR files.
