Facility Evaluation Report

Licensing Program Analyst (LPA) Nakagawa arrived unannounced to continue the Annual Inspection from 07/14/2026. LPA met with Administrator Linda Bibaeff Maldonado. There were 5 staff, 2 volunteers and 11 clients on site at the time of inspection.

LPA found the facility to have a well-organized and multi-faceted activities program which takes place from 9AM to 3 PM Monday through Friday, providing multiple levels of activities to meet each client's specific needs and areas of interest. At the time of visit staff were engaged with clients doing art projects, viewing a video of an artist painting, socializing over breakfast and participation in Tai Chi. Lunches are also served.

LPA reviewed 4 staff and 5 client files and found them to be complete. LPA requested Staff Trainings be kept in an easily accessible format for review.

LPA and Administrator discussed how often medical assessments should be updated as well as the format for Needs and Services Plans.

No deficiencies were found. No citations issued.
