Facility Evaluation Report

Licensing Program Analyst (LPA) Jill Nakagawa arrived unannounced on 05/15/2026 at approximately 2:20 PM and met with Administrator Linda Maldonado Bibaeff. There were 13 clients attending at the time of inspection: there are 23 clients enrolled, who attend on varying days. There were 4 staff and 1 volunteer on site at the time of visit

Administrator was excited to share the facility's new outdoor activity pavilion. The space has walkways, a covered stage, plenty of shade and a storage shed. The area is enclosed by a 6 foot high wrought iron fence which is gated, beautifully landscaped with small shrubs and bark. The area will be supervised by Program staff when clients are accessing the pavilion.

LPA discussed the submission of a new facility sketch to include the new activity pavilion.
