Facility Evaluation Report
Licensing Program Analyst (LPA) Jill Nakagawa arrived unannounced on 08/19/2025 at approximately 9:30 AM to perform a 1 Year Required Inspection. LPA met with Administrator Linda Maldonado Bibaeff. The facility operates Monday through Friday from 9 AM to 3 PM. The Program provides lunch through a partnership with Meals on Wheels, as well as snacks and beverages.
There were 13 clients attending at the time of inspection: there are 26 clients enrolled, who attend on varying days. There were 4 staff and 1 volunteer on site at the time of visit.
The main dining/day room has multiple tables to accommodate an array of activities: puzzles, art projects (today clients were making a hibiscus on "Rainbow Paper"), play games, watch a slide show or read the paper. The room also has a grand piano and a variety of drums, a gong, and other instruments.. The kitchen has a locked pantry. Sharps are locked in a toolbox kept in a kitchen cupboard. The laundry room houses the day-to-day cleaning supplies, which are stored behind a locked door, inaccessible to clients. There was a supply of hygiene products and paper products available for clients, as well as a supply of PPE stored in a separate locked cleaning supply closet, used by cleaning staff. All exterior doors are alarmed with all exits free from obstruction. The facility was clean, well-organized and a comfortable temperature of 73 F. The back yard was designed for ample mobility, with shaded pathways and safe water features. The facility has a central fire alarm/sprinkler system which was inspected on 04/02/2025. There are 4 fire extinguishers which are fully charged and dated 12/02/2024. The last fire drill was held on 04/02/2025.
