Facility Evaluation Report

On 02/24/2026, Licensing Program Analyst (LPA) Nakagawa arrived unannounced to conduct a case management visit regarding the ongoing eviction procedure for client (C1). LPA met with Presley Phillips, Manager Support Staff. Administrator Stacy Sharp was off-site at the time of visit. There were 9 clients and 6 staff on site at the time of visit.

Appropriate placement for C1 is being worked on by Regional Center. The facility continues to work with client, conservator, and the Regional Center to support the client in the interim and to protect the personal rights of all the clients in residency.

LPA also toured the common areas of the facility and found them to be clean, sanitary and a comfortable temperature. The kitchen was in full operation with clients making their lunches for the following day. The living room was clean and comfortably furnished.

No deficiencies found at the time of visit.
