Facility Evaluation Report
Licensing Program Analyst (LPA) Nakagawa conducted an unannounced Annual inspection and was greeted by (3) carestaff and newly assigned Administrator Lucille Calimpon There were 7 residents at the time of inspection. The fire clearance is for 8 residents, 2 of which may be bedridden.
LPA toured both the interior and exterior portions of the facility. Exit doors are alarmed and functional. Disaster drills are conducted regularly. The last fire/evacuation drill was conducted on 05/06/2026. Fire extinguisher was charged and last serviced on 10/06/2026. Smoke detectors were functional and 2 carbon monoxide detectors were present and functional: tested monthly. Hot water measured between 114.8 and 117.8 degrees F in faucets used by residents which is within regulation of 105 & 120 degrees F. There was a sufficient supply of cleaning supplies and hygiene products available for residents. LPA observed adequate perishable and non-perishable foods as required per regulation. Facility was found to be a comfortable temperature of 74 degrees F. Toxins were observed stored and locked in garage. All knives and other sharp items were secured in a locked drawer in the kitchen. Medications are centrally stored in a locked cabinet in the dining room. The facility has an outdoor patio complete with umbrella, table and chairs for residents and visitors to enjoy. There were supplies for art projects in the dining room, along with plenty of games. Residents enjoy walks to the nearby park with staff during good weather.
5 Resident and 3 Staff records were reviewed and found to be complete.
Emergency/Disaster Plan and Infection Control Plan had recently been reviewed. Proof of Liability Insurance provided.
