The inspector’s account

On 8/20/2025, Licensing Program Analyst (LPA) Triel Ellen Lindstrom arrived at the facility unannounced to conduct a required 1-year annual visit. LPA Lindstrom was met by Program Director Micah Sutton and Administrator Deffria Bass-Nwoffiah. The LPA introduced herself, explained the purpose of today's visit, and an interview followed. The Program Director and Administrator accompanied the LPA on a tour of the facility

LPA Lindstrom toured the facility and observed that it was clean, odor-free, and pest free. The building is spacious and brightly lit, with colorful carpet and big windows. The thermostat was set at 78 degrees Fahrenheit. Each room was labeled with signage in English and braille. The facility had many features that made the physical plant and its services ADA (Americans with Disabilities Act) accessible. Some of these features included press pads to open doors, raised garden beds, and wide, level walkways.

The LPA observed the facility’s fire detection system. There were several alarm pull stations throughout the facility connected to an alarm box in a utility room. The system was last serviced by Sentry Alarm on 4/16/2025. Each room had an Emergency Exit Plan posted near the doorway. The facility’s fire extinguishers were last serviced by Cisco Fire Sprinkler Co. on 5/15/2025.

The LPA toured the kitchen classroom and observed that cleaners and sharps were stored in locked cabinets.

The LPA toured the facility grounds. There was a large shade structure on the side of the building with six tables and chairs beneath it, surrounded by raised garden beds that program participants tend. There was a small shade structure with seating on the other side of the facility. There were three storage sheds located in the parking lot. The LPA observed that these sheds contained landscaping tools and day program supplies that are locked and inaccessible to consumers..
