Facility Evaluation Report
Licensing Program Analyst (LPA) Jason Lund arrived announced to the day program today and met with Program Manager Michelle Monahan and explained the reason for the visit. Census: 19
LPA Lund & Program Manager Michelle Monahan toured/inspected the physical plant including the classrooms, bathrooms, kitchen area and activities area. The kitchen areas had refrigerators and microwaves for the clients use as well. Cleaning supplies were observed to be inaccessible to clients and dangerous items such as knives are locked away and inaccessible to clients. All client restrooms are maintained with soap and paper towels available for the clients. Hot water temperature was observed to be 107.5 degrees F.LPA observed the clients actively engaged in activities during this visit. The common areas used for clients were odor-free and all clients appeared to be in the appropriate level of care. Three clients take medication. Medications properly stored, locked and inaccessible to clients. The Daily Medication Administrator Report (MAR) was reviewed and determined to complete and in compliance.
Fire extinguishers were serviced in June 2026 and are charged and in compliance. Smoke detectors and fire alarm systems were tested and are operation. Fire drills are conducted monthly. Carbon monoxide detectors are installed in the building. LPA Lund reviewed three staff, and three clients’ records were reviewed and in compliance. All staff had the required first aid and training.
No deficiencies were identified or cited on this inspection. Exit Interview held and report left.
