Facility Evaluation Report

On 5/02/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced annual required inspection. LPA met with Administrator, Tiffany Johnson and House Manager, Kristal Gomez to conduct facility inspection.

Residents were on a community outing at time of inspection. Facility toured inside and outside. Facility observed to be clean, odor free, and a comfortable temperature. Facility tour began in resident bedrooms, all bedrooms observed to have required furnishings available. All common areas of facility have adequate seating available for residents in care. Resident bathrooms toured, fixtures observed operational. Slip resistant mats available, water temperature measured at 108 degrees F. Kitchen toured, all knives observed to be locked and secured in kitchen drawer. Facility observed to have a 2-day supply of perishable food and a 7-day supply of non-perishable food supply available.

Outside of facility toured, all exits observed to be free of obstruction. Facility has a fenced perimeter backyard. Shed observed to be locked, secured, and inaccessible to resident. No hazards observed.

Fire extinguisher is current with a purchase date of 2/05/2026. Smoke detectors and carbon monoxide detector observed operational. Last fire drill conducted on 4/12/2026. Cleaning supplies observed to be locked and inaccessible to residents in care. LPA reviewed client and staff files. Medications observed to be locked and administered as prescribed. All client files and samples of staff files reviewed to have all the required documents.

No deficiencies issued during today's inspection.

LPA is requesting the following documents be submitted to the Fresno CCL office by 5/15/2026: Designation of Administrative Responsibility (LIC308), Personnel Report (LIC500) and Register of Facility Clients/Residents for LIC9020.

Exit interview conducted. A copy of this report was discussed and provided to Administrator whose signature on this form confirms receipt of this document.
