Facility Evaluation Report
On 4/14/2025, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required Inspection. LPA arrived, introduced self, and allowed entrance by House Manager, Kristal Gomez. LPA met with Administrator, Tiffany Johnson to conduct facility inspection.
Facility is 5 bedroom, 3 bathroom home. All resident bedrooms are private. There are four (4) residents in placement, all were at Day Program at time of inspection. Day Programs hours are Monday through Friday 8:30 AM - 2:30 PM.
LPA conducted facility tour with Administrator. LPA observed facility to be clean, odor free, and a comfortable temperature. Resident bedrooms toured, all bedrooms observed to be clean, have required furnishing. Resident bathrooms toured, fixtures observed to be operational. Shower/tub observed to have non-skid surface and grab bars available, water temperature measured at 116 degrees F. Kitchen toured, facility has adequate food supply for residents in care. All knives observed to be locked and secured in kitchen drawer. Dining room and living room observed to have adequate seating for all residents in care. Medication observed to be locked and secured in medication cabinet which is located in dining room.. Medication observed to have original labels and to be administered as ordered.
Smoke detectors and carbon monoxide detector observed operational during inspection. Fire extinguisher present with a purchase date of 1/17/2025. Last fire drill conducted 1/17/2025 and last earthquake drill conducted 2/14/2025. All chemicals are locked and secured in laundry room,
Outside of facility toured. All exits open free of obstruction. Perimeter of back yard is secured with a fence and free of obstruction. Shed in the backyard is locked and inaccessible to residents. Facility has a shaded area with seating available for residents.
Staff and resident files reviewed. LPA requested updated LIC 500 (Personnel Report), LIC 610 (Emergency Disaster Plan, and LIC 9020 (Register of Facility Clients/Residents) to be submitted to Fresno Regional Office no later than 5/02/2025.
No deficiencies cited. Exit interview conducted. A copy of report provided for facility records.
