On 5/20/2025 Licensing Program Analysts (LPAs) M. Garza and L. Salazar arrived at the facility for an unannounced annual visit. LPAs contacted Licensee, Kathy Lewis who stated staff would be out. Administrator, Tami Leyva arrived some time later. LPAs introduced self, explained reason for visit and were permitted entry into the facility. Residents not present during visit. LPA Salazar conducted resident records review and observed records to be complete with required forms. LPA Garza will document the physical plant and operations on a separate report. No deficiencies cited on this report.
