Facility Evaluation Report

On 4/04/2023, Licensing Program Analyst (LPA) K. Kaur arrived unannounced at the above facility to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and requested to meet with Administrator. LPA was allowed entry by caregiver Lisa Gonzalez. LPA toured facility with caregiver. Administrator Brianna Garcia arrived later during tour. Two clients were present during tour. Administrator Tiffany Johnson and house manager Esmeralda Escalera arrived to assist with the rest of the visit since AD Garcia would be returning to handle an appointment with a resident.

The facility was observed to be at a comfortable temperature, clean, in good repair, no passageway obstructions or fire hazards were observed. Common areas were properly furnished and well-lit throughout. LPA observed sufficient seating in the living room and dining room. Hand sanitizer was readily available to residents and visitors. Sharp items are locked in kitchen cabinet. Cleaning supplies were locked and secured in laundry room. LPA observed a 7-day supply of non-perishable foods and a 2-day supply of perishable foods. Fire extinguisher in the laundry room was purchased on 2/16/2023. Medication locked in the dinning room cabinet.

LPA observed 4 bedrooms that are single occupant. Clean linens and towels observed in the hallway closet. All client’s bedrooms toured and observed to be adequately furnished and lit. Restrooms observed to be clean, odor free and functioning at time of visit. The exterior tour was conducted. Backyard observed to have sufficient seating. Backyard gate was self-latching and self-closing. Medication was reviewed with the centrally stored log and MARs.

Staff records were reviewed for good health, training and first aid requirements. All clients’ records reviewed to have updated emergency contact information, admission agreements, physicians’ orders. Last fire drill conducted on: 03/12/23.
