Staff did not provide transportation to doctors appointments as agreed

Staff did not provide resident records upon request

An unannounced Complaint visit was conducted on the date & times indicated above by Licensing Program Analyst (LPA) K. Mcclurg. LPA met with Administrator (Admin) Tamara Leigh & Program Manager (PM) Veronica Clifton.

Allegations reviewed & discussed with Admin & PM during this visit. Facility provides transportation for medical & dental appointments for clients, however not all clients utilize this service or do not provide notification. Copy of clients reports provided as requested as available.

The Department has investigated the above allegations & determined them to be Unsubstantiated.
