This is an official report of an unannounced visit/investigation of a complaint received in our office on

07/02/2025 and conducted by Evaluator Jacques Leffall

COMPLAINT CONTROL NUMBER: 24-AS-20250702165829

Staff do not ensure clients are provided comfortable accomodations

Staff do not allow clients to make decisions about their care

Staff limit clients ability to participate in community activities

Staff do not follow client's special diet

Staff do not give clients Individual privacy in the bathroom

An unannounced Complaint visit was conducted by Licensing Program Analyst (LPA) J.Leffall. LPA met Assistant Administrator (AA) Veronica Clifton, greeted AA, stated purpose of visit, & was allowed to proceed with visit. The purpose of the visit is to deliver findings of an investigation previously conducted.

The department conducted interviews with staff.

Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did occur, therefore the allegation is Unsubstantiated. No deficiencies were issued.

Exit interview conducted. A copy of this report was distributed to Administrator which confirms signature of this report.
