05/27/2025 and conducted by Evaluator Jacques Leffall

COMPLAINT CONTROL NUMBER: 24-AS-20250527134726

Staff are not preventing residents from using illegal substances while in the facility.

An unannounced Complaint visit was conducted by Licensing Program Analyst (LPA) J.Leffall. LPA met Assistant Administrator (AA) Veronica Clifton, greeted AA, stated purpose of visit, & was allowed to proceed with visit. The purpose of the visit is to deliver findings of an investigation previously conducted.

The department reviewed client records and conducted interviews.

Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did occur, therefore the allegation is Unsubstantiated. No deficiencies were issued.

Exit interview conducted. A copy of this report was distributed to Administrator which confirms signature of this report.
