10/07/2025 and conducted by Evaluator Kelly J. McClurg

COMPLAINT CONTROL NUMBER: 24-AS-20251007090215

An unannounced Complaint visit was conducted on the date above by Licensing Program Analyst (LPA) K. McClurg. LPA met with Case Manager 3 (CM3) Ignacio Garcia. LPA greeted CM3, stated purpose of visit, & was allowed to proceed with visit.

Allegation reviewed & discussed. Resident records reviewed. Facility policy & procedures reviewed.

The Department has investigated the above allegation & determined to be Unfounded.

Exit interview conducted with CM3. Report provided.
