Facility Evaluation Report

An Annual visit was conducted by Licensed Program Analyst (LPA) K. McClurg. LPA met with Assistant Administrator (AA) Veronica Clifton. LPA stated purpose of visit & was allowed to conducted visit.

Front room/dining room appeared to be clean, appropriately furnished, with adequate light. Kitchen maintained. Refrigerator/freezers maintained @ appropriate temperatures. 7 days non-perishable & 2 day perishable available. Client bedrooms sufficiently furnished with adequate lighting. Bathroom fixtures operations & maintained. Wellness room (stand alone building) sufficiently furnished with adequate lighting.

Outdoor areas have seating & covered areas available. Smoking area appropriately identified with signs & appropriate disposal fixture available.

Facility grounds appeared to be well maintained & free of obstructions, hazards, miscellaneous debris, etc. Interior & exterior passageways clear of obstructions & hazards.

Facility, client, & staff records reviewed & discussed during this visit. Emergency & fire drill last conducted 5/7/25. Medications organized & appropriately maintained with appropriate record keeping. Internet available & devices to utilize internet available.

Smoke & carbon monoxide detectors hard wired together & automatically contacts alarm & fire services & maintained by a 3rd party services. Call system maintained by same company, alarm will sound & light outside room lights up indicating which rooms signal originated. Fire extinguisher service date: 7/23/2025.
