08/26/2021 and conducted by Evaluator Kelly J. McClurg

COMPLAINT CONTROL NUMBER: 24-AS-20210826091302

Mental Health Program Manager II (MHPM II) Rosie Ruiz (Maduena)

Facility is not refilling medications as needed;

Facility is not ensuring clients receive health-related services;

Fire extinguishers in facility need to be serviced;

Facility is not providing adequate food service;

An unannounced Complaint visit was conducted on the date & times indicated above by Licensing Program Analyst (LPA) K. Mcclurg. LPA met with Mental Health Program Manager II (MHPM II) Rosie Ruiz (Maduena). LPA stated purpose of visit.

Allegations reviewed with MHPM II, facility toured. Medications. medication records, client, & facility documents reviewed. The Department has investigated the above allegations & have found them to be unsubstantiated.

Exit interview conducted with MPHM II. Report provided.
