Facility Evaluation Report

An unannounced Case management incident visit conducted by Licensing Program Analyst (LPA) K. McClurg. LPA met with Administrator (Admin) Tamara (Cabeje) Leigh.

Purpose of visit was to review R1 records (see LIC811 from 1/25/25) that were removed from this facility on 1/25/25.

In accordance with 80070(d)(3) Client Records:

... Records may be removed if necessary for copying. Removal of records shall be subject to the following requirements:

Licensing representatives shall return the records undamaged and in good order within three business days following the date the records were removed.

Return of records during this is within time frame in accordance with regulation identified above.

No further action needed at this time by the Department.
