Assistant Administrator (AA) Veronica Clifton

Staff are not permitting resident to leave the facility.

Staff are not permitting resident to have visitors.

Staff are not permitting resident to attend religious services.

A Complaint visit was conducted by Licensed Program Analyst (LPA) K. McClurg. LPA met with Assistant Administrator (AA) Veronica Clifton. LPA stated purpose of visit & was allowed to conducted visit.

Allegations reviewed. Facility policies & procedures reviewed. The Department has investigated the above allegations & determined them to be Unsubstantiated.

Exit interview conducted with AA. Copy of report provided.
