08/26/2021 and conducted by Evaluator Kelly J. McClurg

COMPLAINT CONTROL NUMBER: 24-AS-20210826095506

Mental Health Program Manager II (MHPM II) Rosie Ruiz (Maduena)

Administrator is impaired while at work or while on-call;

Administrator is not present enough hours to oversee facility operations;

Facility does not have adequate staffing;

An unannounced Complaint visit was conducted on the date & times indicated above by Licensing Program Analyst (LPA) K. Mcclurg. LPA met with Mental Health Program Manager II (MHPM II) Rosie Ruiz (Maduena). LPA stated purpose of visit.

Allegations reviewed with MHPM II& reviewed facility documents.

The Department has investigated the above allegations & have found them to be unsubstantiated.

Exit interview conducted with MPHM II. Report provided.
