Facility staff failed to notify residents' representatives of positive cases in the facility.

Facility staff are not taking precautions for COVID-19

An unannounced Complaint visit was conducted on the date & times indicated above by Licensing Program Analyst (LPA) K. Mcclurg. LPA met with Mental Health Program Manager II (MHPM II) Rosie Ruiz (Maduena). LPA stated purpose of visit.

Allegations reviewed with MHPM II& reviewed facility documents.

The Department has investigated the above allegations & have found them to be unsubstantiated.

Exit interview conducted with MPHM II. Report provided.
