Facility Evaluation Report

On 10/22/2025 Licensing Program Analyst (LPA) M. Garza completed an unannounced case management visit. LPA met with Program Director, explained reason for visit and was permitted entry into facility. This case management visit is being conducted for an SIR received by the department that occurred on 8/28/25.

During visit LPA completed interviews with Program Director, Staff #1, #2, #3 and Case Manager. LPA also requested, reviewed and obtained copies of DL's, In-service training materials, safety violation notices, and transportation policy.

1) Incident details: During return to the facility after morning on 8/26/25 pick up S1 attempted to park into a stall, scraping the right side of the van on a trailer hitch that was parked in the stall next to the van. There were 7 clients in the van at the time of the accident. Clients did not obtain any injuries in this incident. All RP's were notified. Facility followed policy by providing additional training for S1 and has removed them from the driving duties. At this time S1 is still currently not driving and under review by Transportation Department to see if driving privileges will be returned to S1. Parked trailers have since been removed from the facility parking lot in an effort to limit any additional obstructions.

2) During interviews it was disclosed that there was an additional accident in the parking lot of the facility on 8/20/25. Incident details: After afternoon drop off, spotter (S2) was attempting to guide S3 in backing up the van. S3 did not follow directives that were being given by S2 and backed into a parked truck behind the van. No clients were present in the van at the time of the accident. Facility did not report this incident to CCL. During file review it was noted that S3 had additional accidents on 4/16/21, 11/22/22, and 5/9/23, making this their 4th accident while working for the company. S3 has been removed from the driving duties. At this time S3 is currently not driving and under review by Transportation Department for revocation of driving privileges.
