Facility Evaluation Report

On 12/18/24, Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and met with Program Director’s (PD1 and PD2) Theodosia Johnson and Gloria Waldrum. Administrator Dawn Reynoso was present at facility as well. All clients were participating in Community Integration during the inspection. Client's on behavioral side of facility were present during visit. LPA’s conducted facility tour with PD’s. PD1 toured with LPA on Behavior side of facility and PD2 toured with LPA on Community side of facility.

LPA’s toured the facility. All passageways and exits were clear and free from obstruction. Facility was at comfortable temperature maintained at 70 degrees F. LPA toured The Day Program. The program has 11 classrooms, that include lab room, library, art room, media room, computer club, game room and kitchen. All classrooms and activity areas were clean and odor free. Clients bring their lunches. Kitchen is for small cooking experiences and appeared clean. Food was observed to be stored in refrigerator and freezer. Client’s bathrooms were observed clean, and fixtures were functioning properly. Client lockers were observed in the hallway of facility. Cleaning supplies observed stored and locked in supply closet. Facility quiet room and changing room was toured. Fire extinguisher observed throughout the facility with a serviced date 8/14/2024. Carbon monoxide was tested and observed to be operational. Samples of client files reviewed to have all required documents. Samples of staff’s files were also reviewed to have current First Aid/CPR, fingerprint cleared and associated to the facility.

Exit Interview conducted. The following documents are requested and to be submitted to Fresno CCL by: 1/1/25. LIC 308, LIC 500, LIC 610D, LIC 9283, and LIC 9020. A copy of this report, was provided to Program Director, whose signature confirms receipt of this report.
