Facility Evaluation Report
On 5/28/26 at 2:00 pm Licensing Program Analyst (LPA) J. Leffall conducted a case management regarding an Incident report received by the Department. LPA met with Licensee (L1) Christine Sandoval and stated purpose of visit.
The department conducted interviews and reviewed records. Based on an incident report received by CCLD on 5/26/26 R1 eloped from facility. LPA met with facility to discuss the plan for R1 regarding elopement and AWOL. L1 states the plan for R1 is for R1 to wait inside in the kitchen for Day Program’s staff arrival. Once Day Program staff arrives, R1 will exit the facility and enter the day program vehicle. LPA interviewed R1. R1 states on the day of the incident, another client called him an inappropriate name, hence why R1 did not want to go to day program on the day of the incident. R1 states the day program keeps the 2 clients separated and he likes his day program and likes living at the facility.
No deficiencies is being cited during visit.
Exit interview conducted. A copy of this report was provided to Licensee whose signature confirms receipt of this report.
