The inspector’s account

On 06/24/2025, Licensing Program Analyst (LPA) M. Yang arrived unannounced at the above facility to conduct an Annual Inspection. LPA introduce self, stated the purpose of the visit, and met with Licensee Christine Sandoval. LPA toured facility with Licensee. One client was present upon arrival and left during inspection.

The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed. Sharp items and cleaning solutions are locked under the kitchen sink. Chemicals were observed stored and locked under kitchen sink and laundry room. LPA observed a 7-day supply of non-perishable foods and a 2-day supply of perishable foods. Expired nonperishable food observed. Medications observed locked in kitchen shelf. Medication was checked and MARs was reviewed. Last fire drill completed on 06/10/25. Fire extinguisher was observed with last serviced date of 06/19/24.

Common areas were properly furnished and well-lit throughout. All clients’ bedroom toured and observed to be adequately furnished and lit and restrooms observed to be clean, odor free and functioning at time of visit. Bathroom observed operational. Hot water tested at 117.9 degrees F in master bathroom and 112.6 degrees F in hall bathroom. The exterior tour was conducted. Adequate outdoor seatings observed available for clients. Side exit observed self-latching and self-closing. Pool observed gated and locked. All clients’ files were reviewed to have required documents. Smoke alarms and carbon monoxide tested and operational.

Deficiency is being cited on the attached 809D in accordance with California Code of Regulations, Title 22, Division 6. An exit interview was conducted. Requested documents be submitted to the Fresno CCL office by 06/30/25: Lic 308, Lic 610D, Lic 9020, and current Administrator certificate. A copy of this report and appeal rights was provided to Licensee, who confirms receipt of this report.
