On 3/29/23 at 10:56 AM, Licensing Program Analyst (LPA) Malia Thao arrived uannounced to conduct a case mangement - other inspection. LPA explained reason for inspection and met with Administrator (ADM) Jeff Moyer.
LPA amended LIC809 issued on 2/21/23. No deficiencies cited during this inspection.
Exit interview conducted. A copy of this report was given to Administrator Jeff Moyer, whose signature confirms receipt of this report.
