The inspector’s account

On 07/26/25, Licensing Program Analyst (LPA) L. Salazar arrived to the facility unannounced to conduct the a case management visit based on an incident report dated 06/26/25. LPA was greeted by Administrator, stated the purpose of the visit and was allowed entry into the facility.

On 06/26/25, it was reported by the day program that Resident R1 stated that Staff S1 sprayed R1 in the face with a spray bottle containing chemicals. Administrator was notified and Law enforcement was called. LPA asked R1 if they could be interviewed while at the facility. R1 declined interview.

LPA obtained a copy of R1's file Individual Performance Plan (IPP) and Behavioral Support Plan (BSP). Records review and interviews conducted show R1 has a documented history of false allegations and hallucinations. Staff S1 was removed from the schedule pending investigation and was placed on Administrative leave at the time of allegation and an interval investigation was conducted. Staff S1, Staff S2 , Staff S3, Staff S4 and Administrator were all interviewed and Sheriff's report number was received.

Based on the information received, there is no corroborating evidence that the allegation occurred. No deficiencies cited on today's visit. A copy of this report was provided at the time of visit.
