On 5/25/23 at 8:30 AM, Licensing Program Analyst (LPA) Malia Thao arrived unannounced to conduct a collateral visit. LPA explained reason for inspection and was granted entry. Administrator (ADM) Jonathan Escobedo was not available for the inspection but was available by telephone. ADM gave permission for staff DSP II Rachel Sanchez to sign today's report.
No deficiencies cited during today's inspection.
Exit interview conducted. A copy of this report was left with DSP II Rachel Sanchez, whose signature confirms receipt of this report.
