The inspector’s account

On 8/5/26 Licensing Program Analyst (LPA) M. Garza arrived at the facility for an unannounced case management visit. LPA met with Supervisor, Billy Walker, explained reason for visit and was permitted entry into the facility. Program Manager, Gina Sosa arrived a short time later. LPA observed clients in activity rooms.

This case management visit is being conducted for an incident report received by the Department on 07/29/26.

On 07/28/26, 1:1 staff (S1) was observed asleep inside the van while transporting a client. Management was contacted by driver (S2) for assistance upon return to the facility. Management observed 2 empty bottles of alcoholic beverages inside of S1’s backpack. An odor of alcohol was observed on S1 and client's (C1) breath.

C1 was taken for medical attention and S1 was placed on administrative leave. C1 and S1 were determined by medical staff to be under the influence of alcohol.

Deficiencies cited per California Code of Regulations, Title 22. If not corrected the deficiencies will have a direct risk to persons in care. ***Immediate civil penalty in the amount of $500 assessed***

Exit interview completed with Program Director, Gina and Supervisor, Billy. A plan of correction was developed by PG and Supervisor and reviewed by LPA. A copy of this report, deficiency and appeal rights provided.
