On 8/28/2025, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Case Management visit. LPA met with Administrator, Ashley Newlin during visit. .
The purpose of this visit is to verify that Staff (S1) is not on the property. Facility was provided a copy of decision and order by Department.
Per District Manger, Jose Marques, S1 is not on schedule to work and was disassociated from facility on 8/27/2025.
Exit interview conducted and a copy of this report provided for facility records.
