Facility Evaluation Report

On 04/02/2025, Licensing Proram Analyst (LPA) L. Salazar arrived at the facility unannounced to conduct a Health and Safety visit based on a reporting of death. LPA was greeted by District Manager, stated the purpose of the visit and was allowed entry into the facility. Also present during the visit was Regional Director, Mauricio Villatoro, Administrator Maria Cervantes and Assistant Administrator, Andrue Quiroz.

LPA toured the facility inside and out and observed 3 residents in care at the time of visit. 9 staff members are present at today's visit. LPA observed the required 7-day supply of non-perishable food and 2- day supply of fresh perishables.

LPA requested and obtained the following information: Staff schedule for the February and March 2025, proof of staff training for all staff present today during today's incident (04/02/25). Training records include CPR, CPI and RBT training. LPA obtained Resident R1's Individual Performance Plan and Individual Behavior Support Plan (IBSP). LPA obtained a copy of the Death Report and Incident Report, LIC 602 was not available at the time of visit and will be sent via email. No deficiencies cited on today's visit.
