This is an official report of an unannounced visit/investigation of a complaint received in our office on

04/09/2021 and conducted by Evaluator Kelly J. McClurg

COMPLAINT CONTROL NUMBER: 24-AS-20210409102750

Facility staff did not adequately supervise resident.

Resident was found covered in urine and feces.

An unannouced Complaint visit was conducted by Licensing Program Analyst (LPA) K. Mcclurg. LPA met with Executive Director (ED) Martin Vale. LPA stated purpose of visit.

The Department has investigated the above allegations. Based on availble information, it could not be determined that there was a lack of care & supervision of Resdient 1 (R1), therefore the allegations are unsubstanitated.
