Facility Evaluation Report

Licensing Program Analyst (LPA) Shawna Doucette arrived at the facility unannounced to conduct a case management for an incident that occurred on 5/15/25. LPA met with Administrator Mandy Rancour.

LPA reviewed R1's LIC 602. R1 is unable to manage her own medications.

LPA interviewed Administrator. R1's 8 AM pills were administered however facility staff did not ensure R1 swallowed her pills. R1's pills were found by staff in the afternoon. R1 has a history of not swallowing administered pills.

An exit interview was conducted with the Administrator and a copy of this report was provided with appeal rights and plan of correction.
