Facility Evaluation Report

Licensing Program Analysts (LPA) Sarah Hurt and Lisa Salazar conducted an unannounced facility visit to conduct a Case Management-Health check. LPA met with facility Director of Program Services, Mark Michaelian and explained the purpose of today's visit.

LPA's observed Client 1 is at the day program. Client 1 appeared to be well dressed, talkative, and smiling. LPA's collected and reviewed documents from Client 1's file. LPA's interviewed facility staff, and reviewed documents with staff. Facility was not able to provide Physicians report for Client 1. LPA's reviewed attendance records and requested additional attendance documents be provided.

No deficiencies cited or observed during this visit. Exit interview conducted with Director of Program Services, Mark Michaelia and a copy of this report provided.
