COMPLAINT CONTROL NUMBER: 24-AS-20260204103943

Acting Administrator Angel Silva and District Manager Jose Marquez

Staff did not adequately address resident’s inappropriate behaviors.

Licensing Program Analyst (LPA) K. Kaur arrived at the facility announced to conduct an initial 10-day complaint inspection. LPA was allowed entry by Acting Administrator Angel Silva. LPA discussed the purpose of the visit and the elements of the allegations. LPA delivered the following findings.

The Department investigated the allegations listed above. Based on interviews conducted and records reviewed the facility Staff are addressing resident behaviors while ensuring resident rights are not being violated.

Based on these findings, the above allegations are UNSUBSTANTIATED. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the alleged violations did or did not occur, therefore these allegations are unsubstantiated.
