Facility Evaluation Report
Licensing Program Analyst Shawna Doucette (LPA) arrived at the facility unannounced to conduct a Required Annual inspection. LPA was granted entry into the facility by Staff Evelyn Salas. Staff contacted Administrator Jared Sandoval via telephone who gave permission for Staff Evelyn Salas to sign for this visit.
LPA conducted a tour inside and outside of facility. Facility observed to be clean, and at a comfortable temperature. Common areas were furnished well with adequate seating and lighting available. Kitchen toured, appeared clean and safe for food preparation. Food supply checked, LPA observed an adequate supply of food. Resident rooms checked. LPA observed an adequate supply of linen. Hot water measured at 113.8 degrees F. Facility was set at 72 F. Exterior tour conducted, all exits open and free of obstructions. Side gate was observed to be self-latching.
Fire extinguisher serviced on 06/24/2025. Smoke detectors and carbon monoxide detectors observed operational during today’s inspection. Facility has a pull station fire alarm. Last fire drill conducted 06/04/2025. All cleaning supplies are locked and secured in a cabinet in the laundry room. Medications were locked in a cabinet in the laundry room.
LPA reviewed staff and client files. Medications were reviewed. Staff did not have health screening, TB test and current CPR/First Aid.
An exit interview was conducted with Staff and a copy of this report with plan of correction and appeal rights were provided.
