Licensing Program Analyst Shawna Doucette arrived at the facility announced to conduct a case management for facility closure. LPA met with Licensee/Administrator Jose Pira.

Licensee requested to close the facility. LPA toured the facility. There were no residents observed to be residing in the facility. LPA requested and obtained the facility license.

An exit interview was conducted and a copy of this report was provided.
