Licensing Program Analyst Shawna Doucette arrived at the facility unannounced to conduct an annual inspection. LPA was granted entry by Staff Viloria Editha. Staff contacted Licensee/Administrator Jose Pira via telephone who responded to the facility to assist with the visit.
During the course of the inspection Licensee/Administrator informed LPA all residents were being moved to another facility 5/16/25. LPA will return on 5/17/25 to close the facility.
An exit interview and a copy of this report was provided to Administrator.
