Facility Evaluation Report

One June 2, 2026 Licensing Program Analysts (LPAs) B. Miranda and A. Porter arrived at the facility unannounced to conduct a case management visit regarding an incident previously reported. LPAs met with Administrator Argelia Martinez.

LPA was informed S1 is currently on leave and will not be returning to this facility. Administrator stated at this time S1 is not working in any of the MARS facilities.

LPA is requesting the following documents: Employee roster with contact information (provided during visit), resident roster with contact information (provided during visit), current resident right's training, May staff schedule for the week of incident (provided during visit), R1'2 current IPP, and R1's current physician report. Documents will be provided to the Department by June 4, 2026

Exit interview was conducted and a copy of this report was provided to Argelia Martinez- Administrator.
