The inspector’s account

On 1/12/26 at 11:30 am Licensing Program Analyst (LPA) J. Leffall arrived at the facility to conduct an unannounced Annual inspection. LPA was greeted by Dennis Froshour. LPA stated the purpose of the visit. Per Mr. Froshour, the facility has closed as of 11/29/25 and all of the residents have moved out.
