Facility Evaluation Report

Licensing Program Analyst (LPA) Daiquiri Boyd made an unannounced visit to the facility for the purpose of reviewing a recently submitted incident report and completing a Case Management.

LPA met with Administrator (AD) Eric Morales and Leah Martinez, Program Liaison Assurance Development(PLAD). LPA reviewed a report regarding a recent incident that occurred on 4/12/26 between two residents (see Confidential names list LIC811). LPA gathered copies of both resident's LIC602 and PLAD emailed LPA copies of additional information from the resident files, including Regional Center documents.

LPA found that staff acted to intervene between the two residents, who have a history of altercations with each other. AD explained that the staff met after the incident to debrief and discuss the incident and how things may have been better resolved, but realized that the best actions had been taken.

Resident R1 has asked to be moved and has the support of his Case Manager at Golden Gate Regional Center (LPA was sent these documents by email by PLAD.)

LPA made observations and completed a file review, there are no deficiencies at this time. LPA may return at a later time and further investigation may be warranted for this incident.
